Service Description

Prior to your initial consultation you will be required to complete an in-depth health questionnaire. This will be reviewed during your initial consultation along with your health history. If you have any recent test results then you will be asked to provide these prior to the first consultation along with a list of any medications and supplements you are taking. Home visits allow for a more accurate assessment of environmental factors and is more convenient for patients with mobility challenges.
